During it’s recent Representative Council on 5/4/20, the MHSAA has made an adjustment to the pre-participation physical exam requirement for students participating in athletics during the 2020-21 academic year.
In an effort to alleviate some of the burden caused by the COVID-19 pandemic, the MHSAA pre-participation sports physical requirement will be waived for students in any grade who received a valid physical for sports during the 2019-20 school year (one completed on or after April 15, 2019).
- Students with a valid physical for sports during the 2019-20 school year and their parents/guardians will be required to complete the two-page MHSAA ANNUAL SPORTS HEALTH QUESTIONNAIRE found on MHSAA.com. These must be signed by student and parent/guardian and kept on file in the school.
- Incoming freshman coming to MSHS from BMS who had a valid physical form on file for 2019-20 need only to complete the MHSAA ANNUAL SPORTS HEALTH QUESTIONNAIRE.
- Students who did not receive a sports physical in the 2019-20 school year or later must have a valid physical examination form on file. Click here for the MHSAA PHYSICAL EXAMINATION FORM.
